Third molars, or wisdom teeth as they are commonly referred to are a set of four teeth that erupt into the back corners of the mouth from the ages of 17 to 21. These teeth more often than not will come out impacted, meaning there is no space for them to erupt into the mouth. Impaction may be due to soft tissues (i.e. gums), or hard tissues such as other teeth or bone. Teeth that become impacted are generally more likely to cause problems.

Why take out all four wisdom teeth?

Not only does the impacted tooth need to be extracted but so does its opposing tooth. Teeth will keep moving until they touch their opposing tooth. So if only one tooth is extracted then the opposing tooth will keep moving or over erupting until it eventually hits the gums, eventually leading to more problems.

Having the two opposing teeth out at the same time not only saves the extra visit for removal but also reduces down time from work for healing to take place.

Common problems with wisdom teeth

When an impacted wisdom tooth starts to push through the gums, an infection can start around the top of the tooth resulting in an inflamed small flap of gum. Infection and inflammation (swollen red gums) can cause pain, swelling and jaw stiffness. Infections are by far the most common problem, and although they can respond to antibiotics, the only real way to treat it is to remove the source of the problem.

Damage to neighboring teeth
An impacted wisdom tooth may keep pushing against the molars next to it. This can create food traps and make it hard to clean between these teeth often leading to serious damage to both teeth.

Crowding
If the wisdom tooth comes out angled or putting pressure on nearby teeth, it may push nearby teeth out of their correct position however they are not believed to be associated with crowding of the lower front teeth.

Sacks of fluid called cysts can form around the tooth and may displace the tooth. These cysts can destroy bone and damage other teeth and gums.
